As a member of a larger team, you will oversee a portfolio of clients, ensuring that you provide the necessary support for them to safely stay at home while working towards their goals.To be successful, you will require:Assessment skills: conduct thorough assessments of individuals'' needs, including physical, emotional, and social well-being, gathering information about the condition, medical history, support network, and specific requirements to determine the appropriate level and type of care.Care Planning: develop comprehensive care plans tailored to each individuals needs and preferences, considering medical treatments, personal assistance, emotional support, and social activities to ensure holistic care.Consumer-Focused: place the consumer at the centre of work, prioritising choices, preferences and goals, and involve the individual in decision-making.Liaison with Health Professionals: act as a liaison between the consumer and the healthcare system, collaborating with doctors, nurses, therapists, and social workers to implement care plans and manage referrals as needed.Visiting Consumer: conduct home visits to assess well-being, monitor progress, and provide ongoing support, building rapport and ensuring quality of care.Empathy and Communication: demonstrate strong empathy and communication skills to listen actively and build trust with the consumer and their family.Problem-Solving and Advocacy: identify challenges, develop solutions, advocate for the consumers rights, and navigate obstacles to access appropriate care.Organisation and Time Management: manage multiple cases, prioritise tasks, and meet deadlines effectively.Compassion and Patience: maintain a compassionate, patient approach to support individuals and families through challenging situations.What were looking for:Appropriate qualifications compliant with Australian Qualifications Framework (e.g., Diploma of Case Management and/or Community Services/Nursing or related field)A current Victorian Drivers LicenceA current Working with Children Check (Employee) or VIT RegistrationFor more informationContact: Kristy Pearson, Care Partner Team Leader, 1800 242 696Latrobe Community Health Service is committed to better health, better lifestyles, and inclusive communities.
