Social Worker Aboriginal Health Case Manager (Devonport, TAS) Location: Devonport, TAS Employment Type: Full-time / Part-time (Negotiable) Salary: $35$45 per hour + salary packaging About the Role MMT Recruitment is seeking a dedicated Social Worker on behalf of a highly respected, community-based healthcare provider in North-West Tasmania. In this vital role, youll support individuals experiencing severe and complex mental health challenges through personcentred case management and care planning. This is a nonoutreach, officebased and communityfacing position within a multidisciplinary mental health team. You''ll have the opportunity to build strong therapeutic relationships and provide coordinated support that empowers clients to improve their mental wellbeing and navigate life more confidently. Key Responsibilities Conduct clinical assessments and develop personalised care plans Coordinate with GPs, psychiatrists, psychologists, and allied health professionals Support client advocacy during medical and social appointments Engage with family and carers where appropriate Maintain clear documentation and participate in team meetingsContribute to health promotion, community engagement, and recoveryoriented care Requirements Degree in Social Work or related discipline 25 years experience in community mental health or social support roles Strong understanding of psychosocial support principles and traumainformed care Excellent communication and stakeholder engagement skills WWVP check and valid drivers licence essential Must have full Australian work rights Why Apply Through MMT Recruitment?
