Social Worker St John of God Health Care, Midland Hospital We are looking for a dedicated and compassionate Social Worker who is a trained ACAT/ACA Assessor to join our multidisciplinary team. In this role you will provide highquality clinical social work services to clients and patients, using evidencebased practice to support wellbeing and independence. Responsibilities Demonstrated clinical knowledge and skills in assessment, planning, implementing and evaluating Social Work interventions. Provide Social work services to DVA, TAC and Workcover patients. Actively intervene with counselling, support, education, community liaison and referrals. Lead family meetings where appropriate. Collaborate within a multidisciplinary service approach; contribute to evaluation of the Social Work service and Clinical Specialty. Deliver patientcentred care, build health literacy and promote selfmanagement. Maintain reflective practice and commitment to professional development. Communicate issues and ideas for enhancing the service. Maintain knowledge of supported accommodation and nursing home facilities and eligibility. Ensure timely referrals to community Health Care Providers. Liaise effectively with community agencies and external Health Care Providers. Maintain knowledge of legislation and policies relating to discharge, DVA and compensable patients. Conduct quality audits to improve patient care. Attend relevant training and complete all mandatory competencies. Position Requirements Applicants must hold a relevant tertiary qualification in Social Work and be eligible for membership with the Australian Association of Social Workers (AASW). They should demonstrate strong experience in social work assessment, case management and discharge planning, applying these skills to support clients in achieving positive outcomes. Excellent interpersonal, verbal and written communication skills are essential. Benefits Salary: $85,508 $120,255 per annum, plus 12% superannuation.
