Clinical Assessment Officer Merri-bek City Council The Early Years and Youth Branch is seeking a part-time Clinical Assessment Officer. The position is responsible for undertaking comprehensive assessments in clients homes for people that are 65 years or older (50 years or older for Aboriginal and Torres Strait Islander people) who require assistance with everyday tasks. Working as part of a consortium under the Single Assessment System (SAS) the role will collaborate closely with consortia partners and their staff to ensure consistency and a streamlined, clientfocused assessment process. The successful candidate will demonstrate excellent communication, interpersonal and problemsolving skills, and the ability to manage their own workload. Key Qualifications Tertiary qualification in a healthrelated discipline directly related to health, aged care or a specialist area such as Allied Health or Social Work (current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA) or other relevant professional association). Minimum of 3 years of demonstrated experience directly delivering services in aged care settings and/or to aged persons (including geriatric evaluation, rehabilitation, palliative care, community nursing), including working with people living with dementia and in community settings with culturally and linguistically diverse communities. Demonstrated experience or knowledge of aged care assessment. Demonstrated experience or an understanding of My Aged Care and Commonwealthfunded aged services. Understanding and experience using the Integrated Assessment Tool with clients. Current drivers licence and Working with Children Check.
