HealthWISE is seeking a dedicated Care Manager to coordinate and oversee Diabetes Shared Care Clinics in Narrabri. In this role, you will provide clinical oversight, general health checks, and support the delivery of high-quality, multidisciplinary care to clients in the region. About the Role Coordinate and oversee fortnightly Diabetes Shared Care clinics Provide general health checks and holistic client care Work within a multidisciplinary team to ensure integrated service delivery Support referral pathways, case conferencing, and health education activities Build strong community and stakeholder relationships About You Registered Nurse (RN), Enrolled Nurse (EN) or Aboriginal Health Practitioner (essential) Ability to work independently and within a multidisciplinary team Strong communication and organisational skills Willingness to travel and work flexibly as required Experience with diabetes care (desirable) Why Join HealthWISE?
