As a Clinical Pharmacist, youll work closely with Clinicians, Nurses, and Technicians, manage specialised medications and contribute to patient care from admission through to discharge. Occasionally, you may be required to work at Slade Pharmacy Kempsey, located within Kempsey District Hospital, approximately a 42minute drive from Port Macquarie Base Hospital.Key ResponsibilitiesConducting medication chart reviews and medication reconciliationDispensing and supplying medication and pharmacy productsCoordinating patient medication requirements for inpatient stay, dayinfusions and dischargeDeveloping and maintaining professional relationships with key stakeholders to ensure excellent customer service to clients and patientsSupporting patients, carers, hospital staff and associated employeesEducating and advising staff and customers on health promotion, disease prevention, drug selection and the quality use of medicinesWhat Youll BringWere looking for Pharmacists who thrive in a collaborative clinical environment, with strong communication skills, attention to detail, and a patientcentred mindset.
