These NSW Government-funded initiatives help people aged 16+ with serious mental health challenges to live independently and meaningfully in the community.In partnership with Local Health Districts, these programs offer **flexible, recovery-focused support** that adapts to each persons needs, empowering individuals to build skills, regain independence and thrive.**About You**In this rewarding role, youll provide hands-on, practical support with daily living skills, attending health appointments and engaging in community activities. Youll assist people to reconnect with education, employment and stable housing, while also supporting smooth transitions back into the community from hospital or prison. Central to your work will be building strong, positive relationships with families, friends, service providers and local networks to foster connection and inclusion.Any experience in working with linguistically diverse groups including Aboriginal and Torres Strait Islander, refugee groups and LGBTIQ groups will be highly regarded.Youll bring a contemporary understanding of mental health and recovery-oriented practice, along with a deep commitment to empowering individuals with complex needs to achieve their personal goals. With a flexible, person-centred approach and strong communication skills, youll thrive in our team environment.
