Youll ensure clients have the right supports in place for independence, choice and control, while also promoting the benefits of Care Connects wider services.In this role, you willReview referral and intake information, completing client assessments (face-to-face or via phone) within agreed timeframes and KPIsDevelop person-centred care plans - identify needs, explain rights & responsibilities, and set client expectations collaborativelyBuild strong referral networks and connect clients with local resources aligned to their care plans and budgetsProvide service excellence across all stages of the client journeyAbout YouYou thrive on engaging with people, value relationships, and are confident in delivering person-centred care to a diverse client base.
