TeamOur NWRH Social Workers team practices collaboratively with the wider multidisciplinary team to support vulnerable people experiencing an acute medical event and likely to require lifestyle adjustment, inpatient discharge planning, and identified social care needs during and post their hospitalisation. Reporting to the manager of Social Work services, our dynamic and supportive team provides psychosocial support to inpatients and outpatients at both hospital sites and supports communitybased social workers who focus on hospital avoidance, wellness and reablement, and groupwork support through Primary Health funded services. Our team proactively engages with the multidisciplinary team and external agencies to ensure best outcomes for our patients, promotes a culture of shared learning and respect for diverse perspectives, and works cohesively across acute and community services.RoleAs a member of the Hospitals North/North West Social Work team, you will contribute to the development, provision, and evaluation of social work services for patients of the hospital and their families and/or the community in accordance with the Code of Ethics of the Australian Association of Social Workers and the Code of Conduct of the Hospitals North/North West.Practice across all other clinical caseloads including Medicine, Surgery and Emergency.Receive ongoing support and supervision from a Senior Social Worker.Be part of a team built on collaboration, trust, respect and teamwork.You will be required to participate in a weekend on-call Social Work Service. Hours are to be negotiated with the successful applicant. For a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar fulltime, parttime, and casual vacancies.Salary$73,834 to $111,364 per annum (Pro rata). Our Employer 12% superannuation contribution is on top of this amount.
