Job Title: Service Support Coordinator Community Interlink Employment Type: Permanent, Part Time Classification: Grade 2 (HS2, HS18-HS21) At Goulburn Valley Health we pride ourselves on delivering personcentred care. About the Role Community Interlink is a partnership between local hospitals and health services across central and northern Victoria and southern New South Wales. The role supports older people to manage their Home Care Support funding by connecting clients with the services they need, working closely with Case Managers. Key Responsibilities Help clients stay independent and safe at home. Organise services through Purchase Orders. Check in to ensure clients are satisfied with their care.
