About the Role This role requires an experienced Registered Nurse to join the Hospital Admission Risk Program (HARP) Care Coordination team at Northern Health, in a fixedterm maternity leave cover position, commencing December 2025. The role is responsible for delivering safe, highquality care to clients with complex needs and/or chronic disease who are at risk of readmission to hospital. To be Successful You Will Have: Minimum 5 years clinical experience Experience and excellent knowledge in care coordination or case management Clinical leadership in the continuing care setting Demonstrated advanced clinical assessment, intervention planning and delivery skills Demonstrated high level knowledge of community, residential and acute/subacute services that support clients with chronic disease and complex needs Demonstrated high level problemsolving and planning ability Next Steps If this opportunity sounds like the next step in your career, we would love to hear from you.
