About the role As a Registered Nurse in the HITH team, youll deliver highquality, evidencebased nursing care to patients recently discharged from hospital or requiring acute treatment in the community. Youll work closely with medical officers, allied health, and other nursing professionals to deliver services including: Comprehensive nursing assessments and clinical observations Medication administration and reconciliation Wound care and pathology collection Client education, advocacy, and discharge support Coordination with multidisciplinary teams to ensure seamless care This role offers the perfect balance of independence and teamwork a great fit for nurses who thrive on variety, enjoy thinking on their feet, and take pride in providing holistic care outside the hospital setting.
