Working closely with a team of Registered and Enrolled Nurses, youll use your clinical expertise to deliver safe, holistic and client-centred care. Your Responsibilities Will Include Coordinating care plans and supporting client onboarding Providing clinical advice and guidance to field nurses Conducting client assessments and developing tailored care plans Supporting case management using our Sandwai software Monitoring referrals, feedback, incidents and escalations Maintaining wound care supplies and consumables What Were Looking For 35 years experience as a Registered Nurse (community nursing desirable) Strong clinical and risk assessment skills Competence in recognising client deterioration and implementing strategies Excellent communication, organisation and time management skills Ability to work both autonomously and collaboratively Requirements Bachelor of Nursing with current AHPRA Registration National Police Clearance & DHS Clearances (NDIS, WWCC) Current First Aid, CPR and Manual Handling certifications Current Drivers Licence About Pop Up Health We specialise in providing personalised healthcare services that respect the dignity and independence of our clients.
