About the role: We are looking for a Part-time Registered Nurse to join our welcoming and supportive Juniper Home and Community Care team in the South Perth metropolitan area. In this rewarding role, you will provide care within the community, visiting clients in their own homes across the South Metro region to deliver compassionate, high-quality support. As a Registered Nurse with Juniper Community, you will play a vital role in partnering with clients to assess their needs, develop strength-based strategies, and evaluate outcomes. Your responsibilities will include conducting comprehensive clinical assessmentssuch as cognitive evaluationsand creating tailored care plans. You will collaborate closely with our broader Home and Community Care team, which includes allied health professionals, case managers, and community support workers. Youll be supported in an environment that values initiative, teamwork, and compassion. This is a permanent part time position working every Monday and Tuesday from 0800-1600. Candidates with previous experience working in Home and Community Care will be highly regarded. Key Responsibilities: Assess and review customer needs in consultation with case managers Clinical Nursing assessments (PAS & KICKA) Develop, implement, and evaluate plans of care in partnership with customers and their significant others for both chronic and complex needs Simple & complex wound care Liaise with appropriate health care professionals to deliver quality care to customers Participate in multidisciplinary team meetings Ensure full compliance with organisation, professional and aged care standards Key requirements: Current registration with AHPRA as a Registered Nurse Community experience highly regarded AlayaCare experience highly desirable Ability to relate to elderly customers with empathy whilst working effectively with other health professionals, service providers and case managers Strong clinical assessment skills Work autonomously with strong communication, organisational and time management skills Enthusiasm for professional growth and development Competency with IDCs, wound care and strategies for living with dementia desirable Cultural awareness and sensitivity Current Australian drivers licence and reliable vehicle - both essential Whats on offer: Attractive hourly rate between $44.76 - $56.45 per hour * dependent on experience.
