Mental Health Professional - Forster Continuing Care Team Join to apply for the Mental Health Professional - Forster Continuing Care Team role at NSW Health. Position classification: Occupational Therapist Level 1/2, Social Worker Level 1/2, Psychologist, Registered Nurse Counsellor Level 1/2, Dietitian Level 1/2. About the Role An exciting opportunity exists for a motivated and enthusiastic Mental Health Professional to join the Continuing Care Team within Manning Mental Health Service, based at Forster Community Health Centre. This role sits within an acute community mental health service supporting consumers across the Lower Mid North Coast. As a key member of our multidisciplinary team, you will provide highquality assessment, care coordination, intervention and followup for adults experiencing mental health concerns. You will work collaboratively with consumers, their families/carers, GPs, Community Managed Organisations (CMOs) and other service providers to ensure safe, effective, recoveryfocused care. About me You will be a motivated and enthusiastic worker, able to contribute your knowledge in mental health to be part of a multidisciplinary team. You will be directly supported by the senior leaders, Manning Service Manager and the broader Manning Mental Health team. A qualification in one of: Occupational Therapy, Psychology, Nursing, Social Work, Counselling, Nutrition or Dietetics, with eligibility for membership or registration with the relevant professional body/AHPRA. You will be someone who: Eligibility to drive in NSW and interstate, and willingness to travel for workrelated purposes. Has knowledge and experience in mental health assessment, care planning and contemporary practice. Works in a consumer, family and carerfocused way. Understands and applies traumainformed care. Communicates well and builds strong therapeutic and professional relationships. Works effectively within a multidisciplinary team. Manages competing clinical demands and maintains professional boundaries. Can provide consultation to GPs, CMOs and other service providers. Is committed to ongoing learning, evidencebased practice and service improvement. Brings a positive, collaborative and flexible approach to the team. Additional information This position is full time; however, part time arrangements will be considered.
