Youll be part of a dedicated, values-driven team providing person-centred care to older Australians helping clients stay independent, safe, and connected in their own homes. What You Will Be Doing Deliver holistic nursing care including assessments, care planning, wound and chronic condition management, and palliative support. Collaborate closely with clients, families, and our multidisciplinary team to achieve the best outcomes. Manage your time and schedule efficiently while maintaining a focus on quality care and client satisfaction. Contribute to continuous improvement, always striving to enhance the care experience. What You Will Bring Strong clinical judgment with experience in chronic condition management, wound care, and palliative care. Excellent communication and time management skills, with confidence working both autonomously and collaboratively. Professionalism and reliability, with a genuine passion for supporting older people to live independently. Proficiency with IT systems and administrative accuracy.
