Join to apply for the Clinical Partner role at HammondCare Overview HammondCare at Home is currently looking for an experienced Clinical Partner to support the team at Batemans Bay. The Clinical Partner works within HammondCares Mission in Action to deliver high-quality, person-centered clinical care that supports clients to live well in their own home. The role provides evidence-based, best practice clinical care holistically to individuals, ensuring their unique goals, needs and preferences are understood and addressed. This role is focused on assessing clinical care needs, developing and reviewing care plans and delivering excellent clinical care in partnership with clients and key stakeholders. The role works collaboratively with the care management team to ensure the best outcomes for clients in the community setting and provides guidance to care staff to ensure the safe and continuous delivery of high-quality clinical care. Responsibilities Assess clinical care needs and develop, review and implement care plans. Deliver high-quality clinical care in partnership with clients and key stakeholders. Collaborate with the care management team to achieve optimal client outcomes in the community setting. Provide guidance to care staff to ensure safe and continuous delivery of care. Essential/Mandatory Commitment to engage and align with HammondCares Christian Mission, Motivation and Mission in Action. Nursing qualification and current registration with AHPRA as a Registered Nurse (Division 1, General). Passion for and previous experience in client-focused service delivery in aged and community/home care settings. Knowledge of the Aged and Home Care Reforms and Aged Care Quality Standards. Excellent communication and collaboration skills, with a commitment to advocacy and person-centred care. Ability to work effectively in a multidisciplinary team and independently across multiple sites. A current drivers license and the ability to travel between HammondCares sites and services. Excellent administrative and computer skills, including digital health records and care coordination tools. Commitment to professional development. Empathy, patience, and a passion for delivering high-quality, person-centred care. Desirable Nursing experience in aged care and community care. Ability to identify and implement innovations in care delivery and service improvement.
