Working with external care teams and families, the role is responsible for providing high-quality, responsive, and person-centered support to assist participants in enhancing their everyday living skills, gaining access to and participation in the community, and achieving their goals to improve their quality of life, physical health, and mental health.Supporting clients with personal care, community access, and daily routinesWorking in participants homes or in our purpose-built Vertical Villages (SIL/SDA)Managing complex needs including behaviours of concern, trauma, forensic backgrounds, and high clinical needsAdministering medication and following detailed health support plansImplementing behaviour support plans and using de-escalation techniquesProviding meaningful, person-centred care to NDIS participantsCollaborating with clinicians, case workers, and familiesAbout YouYou will thrive and succeed in our team by bringing the following experience to this position:Minimum 6 months experience in disability, aged care, mental health, or related fieldsCertificate III/IV/Diploma in Disability, Individual Support, Aged Care, or similar (preferred)Experience supporting people with mental illness, high physical needs, and behaviours of concernDemonstrated experience with high physical and clinical needs, including medication administrationStrong understanding of professional boundaries, restrictive practices, and behaviour supportEffective written and verbal communication, including clear case note writingAble to set priorities, meet deadlines, and work under pressure with minimal supervisionStrong interpersonal skills to work both autonomously and as part of a teamNDIS Worker Screening, First Aid, Drivers Licence, and insured vehicleFeel free to apply if you are passionate about helping people live life on their terms.
