Join to apply for the Case Manager Reconnect role at Lutheran Services Australia. This is a fulltime position located in Caboolture, QLD. Remuneration ranges from $45.47 to $48.93 per hour, plus superannuation, salary packaging and staff discounts. About the Reconnect Program: The program supports young people aged 1218 and their families or support networks to navigate challenging circumstances and build sustainable solutions. Reconnect offers counselling, mediation and practical support. At risk of becoming homeless Couch surfing Experiencing family conflict Disengaging from school In need of support to access training, employment or accommodation What Youll Be Doing: Provide case management, counselling and mediation to young people and families. Conduct holistic assessments and create clientled support plans. Deliver evidencebased therapeutic interventions. Assist young people to access accommodation, education, training and community supports. Facilitate group work, lifeskills programs and psychoeducation. Work flexibly, including outreach in the community and home settings. Develop strong connections with local schools, services and referral networks. Maintain accurate case notes, data entry and reporting requirements. What You Bring: Experience supporting young people and/or families through case management. Strong knowledge of youth development, family systems, risk factors and intervention models. Highlevel communication, engagement and deescalation skills. Ability to work autonomously and collaboratively within a multidisciplinary team. Confidence using Microsoft Office and case management systems. Essential: NDIS Worker Screening | Blue Card (Working with Children Check) | QLD Drivers Licence. Desirable: Human Services, Social Work, Psychology or related qualification. Why Choose Us?
