What Youll Do Partner with residents and their chosen supporters to understand what matters to them and codesign wellness, reablement and lifestyle goals under the Support at Home program. Develop, implement and review individual care and support plans that reflect goals, preferences, risks and changing needs, using tools such as care plans, assessments and budget tools. Coordinate and monitor safe, timely, effective and costefficient services that uphold residents independence, dignity and choice. Build resident service models, and prepare care service agreements, ensuring fees and inclusions are transparent and fully discussed with residents and/or families. Act as the key point of contact for changes, cancellations and adhoc service requests, responding promptly and communicating clearly with residents, families and the care team. Regularly review Support at Home budgets to ensure adequate services are in place to meet residents goals while avoiding overspend of allocated funds. Advocate for residents rights and preferences in line with the new Aged Care Act and the Code of Conduct for Aged Care, supporting informed decisionmaking. Build strong, collaborative relationships with the Wellness Manager, clinical team, Community Manager, external providers and the Finance Team to ensure seamless service delivery and accurate billing. Contribute to quality improvement projects, audits and reporting requirements in line with Odysseys Quality Management System and legislative frameworks. Assist with new resident assessments, onboarding and movein documentation as required, and take on additional duties as services grow and evolve. What You Bring Minimum 2 years experience in a case management, care coordination or similar role within aged care, community care or health services. Strong understanding of personcentred, homebased care and the emerging Support at Home framework, with working knowledge of My Aged Care pathways and systems. Good understanding of the Aged Care Quality Standards and the regulatory environment, with an interest in the new Aged Care Act and strengthened standards. Demonstrated ability to build trust with residents and families, manage complex situations, and find practical, residentfocused solutions. Experience planning and managing individual budgets, monitoring spend, and balancing value for money with quality outcomes. Confident using digital systems and technology (MS Office, care management systems, smartphones, tablets and laptops). Clear, confident written and verbal communication skills, with strong documentation capability and the ability to accurately record and track care management minutes in an organised, systematic way. Current First Aid/CPR (or willingness to obtain), influenza vaccination, valid working rights and a satisfactory National Police Check (or willingness to obtain). Desirable Bachelor of Nursing Experience in retirement living, home care packages/support at home, or flexible inhome support services. Knowledge of the Retirement Villages Act QLD Why Odyssey?
