In this role, youll help older people remain living independently in their own homes by providing expert case management and support coordination. Youll work closely with clients and their families to design tailored care plans that meet their needs, goals, and lifestyle. Build strong relationships with clients, families, and service providers. Conduct assessments, referrals, and regular reviews. Develop and manage individualised care plans and budgets. Attend meetings at our Bayswater office and visit clients across the Easter and Southern suburbs. About You Youre a caring, genuine, and proactive individual with a passion for aged care and supporting older people to live well at home. You are confident working independently and have a strong ability to manage and prioritise a caseload. You will bring Minimum 2 years experience in a similar Care Advisor/Case Partner role. Background in nursing (EN/RN) or allied health preferred. Diploma in Community Services or Case Management, or willingness to commence studies. Exceptional interpersonal, negotiation, and time management skills. Strong IT skills and understanding of aged care quality standards. Ability to speak a second language (German, Italian or Greek) is a strong advantage. Requirements Current Victorian Drivers Licence + own insured vehicle. Current Police Check (within the last 12months). Must be an Australian Citizen or Permanent Resident. Ready to make a difference?
