Youll have the option to work from home flexibly and also be out on the road visiting clients in home.Your day to day will includeConducting needsbased assessments and developing personalised care plansCoordinating services in collaboration with healthcare professionals and familiesMonitoring and adjusting care plans as client needs evolveGuiding clients and families through the Home Care Package journeyStaying on top of documentation and compliance requirements with care and accuracyAbout youDemonstrated experience as a Care Manager in Home Care Packages / Support at HomeStrong communication skillsyou build rapport easily and work well with people from diverse backgroundsHighly organised with great time management and attention to detailUnderstanding of the Support at Home programmeConfident with tech, including electronic care management systemsAt Sai, youll be part of a supportive team thats passionate about delivering outstanding aged care.
