Hybrid working enjoy flexibility with home and office days Real career progression opportunities as we continue to grow A workplace that values compassion, respect and collaboration What Youll Be Doing Partner with customers and their families to develop and review personalised Care Plans and budgets Guide customers to confidently manage their Home Care Package funds and make informed choices about their supports Conduct Telehealth assessments, Care Plan reviews, and budget reviews to ensure funds are used effectively Maintain accurate records and meet compliance and regulatory standards Work collaboratively with a supportive Aged Care team that values innovation, teamwork and customer empowerment About You 12+ months of experience as a Care Manager, Case Manager, or Home Care Coordinator, ideally within a Self-Managed Home Care setting Strong understanding of Aged Care, Home Care Packages, and consumer-directed care principles Confident using technology and typing during Telehealth consultations Excellent communication, problem-solving and documentation skills Passionate about supporting customers to live independently and achieve their goals Why This Role Matters Youll be part of a mission-driven team helping older Australians stay connected, independent and in control of their care.
