Arche Health is a not-for-profit primary health care organisation that supports better coordination between primary health care services, identifies gaps in service delivery and helps improve the way primary care is delivered. The Perth Multicultural Healthlinks (PMHL) is a collaborative partnership commissioned by WAPHA. Its core mission is to significantly enhance primary healthcare access and integration for culturally and linguistically diverse (CaLD) / multicultural communities in the Perth North and Perth South PHN regions. Multicultural Health Program Lead The Multicultural Health Program Lead delivers culturally responsive services on the ground, links community members from multicultural or CaLD backgrounds to relevant primary health care services, and supports service integration and coordination at a local level. The position is part-time, 4 days a week, located in Bentley, WA. Applications are for experienced program leads. Responsibilities Oversee and manage day-to-day operations, providing active care navigation services to eligible multicultural clients to improve community and client outcomes. Collaborate with a range of stakeholders to ensure all activities comply with the funding agreement. Represent Arche Health and partner with service delivery partners to ensure effective, efficient operations across designated regions. Convene and manage steering and operational meetings with partners and their representatives to maintain alignment and accountability. Lead and support a team of Health Care Navigators, addressing performance issues in accordance with policies and procedures. Monitor, evaluate, and optimise data collection processes to meet funding body dataset requirements. Oversee recruitment and onboarding of new staff with thorough orientation to service procedures and operations. Ensure complaints and incidents are managed promptly in line with policies and procedures. Undertake additional duties and projects as required to support organisational objectives. Provide practical assistance for multicultural community members to access primary care locally, including health literacy support and selfadvocacy education. Support interpreter and translation services, ensuring access to appropriate communication resources. Engage in community codesign, stakeholder collaboration, champion engagement, and promotion of the service. Conduct monitoring, evaluation, data entry and reporting using the MultiNav software platform. Essential Selection Criteria Qualification in the mental health or welfare sector (CertIV minimum, Diploma, or Degree). Minimum of 3 years'' experience in direct service delivery and team management. Experience working with migrant and refugee communities. Strong cultural competence and ability to deliver culturally responsive services that promote substantive equality. Ability to work with interpreters and in a multilingual environment. Strong community engagement, empowerment and capacitybuilding skills. Excellent communication skills, including advising multicultural community members, general practices and other services. Proven ability to collaborate with a range of stakeholders (service providers, general practices, community leaders). Ability to work independently and within a team; effective time management and organisational skills for managing competing priorities. Sound computer literacy and proficiency in Microsoft Office; accurate data capture in the MultiNav iCRM platform.
