Our client, a respected healthcare provider and health service, is seeking a dedicated Occupational Therapist Grade 2 to join their Rural Allied Health Team based in Shepparton, Victoria. As a valued member of this interdisciplinary team, you will deliver high-quality Occupational Therapy services tailored to frail older people, individuals with disabilities, and their carers. Your role involves conducting comprehensive assessments, prescribing equipment and home modifications, and supporting activities of daily living retraining programs that promote independence. The successful candidate will work within community and home environments applying the principles of the Victoria HACC PYP Active Service Model and CHSP wellness frameworks, focusing on reablement and restorative care. This approach ensures clients maintain participation in social and community activities, delaying or reducing the need for ongoing services. Your responsibilities include collaborating closely with case managers, support coordinators, and planners to tailor care packages aligned with client goals under funding programs such as the Commonwealth Home Support Program, Home & Community Care Program for Younger People, Home Care Packages, and the National Disability Insurance Scheme. Responsibilities Deliver client-centered occupational therapy interventions in line with program guidelines. Participate in integrated assessments and interdisciplinary care planning. Provide expert advice to community organisations and contribute to quality improvement initiatives. Support clinical intake and supervise occupational therapy student placements. Engage in professional development, reflective practice, and team collaboration. Qualifications Bachelor of Applied Science in Occupational Therapy or equivalent Current AHPRA registration Demonstrate experience providing episodic, self-managed occupational therapy services to clients with complex needs across diverse settings.
