The Role We are looking for a Case Coordinator in the Home Care team, who is passionate about a clientcentered approach to care. The Case Coordinator ensures safe, highquality care for home care clients through direct case management and the provision of advice and support to the home care workers. The Case Manager provides a range of support and coordination services aimed at the provision of safe, highquality, consumerdirected home care. You must be dedicated to working in the Home Care Model and be able to apply applicable quality standards, including the Aged Care Quality Standards, NDIS Practice Standards and Attendant Care Standards. What You Will Do Provide case management and support using knowledge of the home care funding streams (e.g. Home Care Packages Program, NDIS Disability Service, Home Care Packages, DVA Community Nursing, Other funding streams that require complex care) Promote and ensure Care Plans are developed in true partnership with clients. Support the investigation and resolution of complaints related to care. Provide coaching and mentoring for carers in relation to care and career development.
