Overview Bridges Health & Community Care Ltd is proud to deliver the new Regional Reset program, part of the Queensland Governments Making Queensland Safe plan funded by the Qld Department of Youth Justice and Victim Support. BHCCs Regional Reset catchment area includes the Wide Bay region (Hervey Bay, Maryborough and Bundaberg with outreach to the North Burnett). Regional Reset supports young people aged 817 who are at risk of entering the justice system due to antisocial behaviour, disengagement from school and/or adverse life circumstances such as poverty or family dysfunction which puts them at risk of misadventure or crime. The program offers intensive support for the young person and their family and, if required, a short stay/livein component to enhance skills in making good choices and identifying positive support networks and resources in the local community. The Position Bridges is seeking experienced family practitioners to assist in the delivery of the Regional Reset program for youth aged 817, funded by the Queensland Youth Justice & Victims Support. These positions are available for commencement in December2025/January2026. Background and Staffing Bridges Health & Community Care Ltd''s Regional Reset (RR) service model is grounded in strong local partnerships, cultural leadership, and decades of experience supporting young people within regional Queensland. The model responds to intersecting challenges trauma, disrupted education, cooccurring mental health and substance use, and early justice involvement through sustained, tailored wraparound support. All team members receive traumainformed training, cultural and general supervision, and reflective practice to ensure safe, strengthsbased support for highrisk young people. The service delivery is coordinated by a multidisciplinary team that includes Team Leaders, cultural advisors, youth mentors, social workers/family practitioners, and operational support staff. This team is responsible for referral management, mentoring supervision, reporting, stakeholder engagement, and quality assurance. Team Leaders ensure quality, risk and compliance in all areas: program fidelity, communication, stakeholder engagement and partnerships, and overseeing data and reporting. Cultural advisors guide the culturally safe delivery. Operational support staff provide administrative support to the RR staff, including processing referrals, capturing data, sourcing supplies and services, and ensuring staff have the resources necessary to fulfil their roles, e.g., access to vehicles, IT, etc. Mentors are the primary point of contact for young people, embedded within a team structure that ensures continuity, safety and accountability. Mentors receive regular supervision, participate in reflective practice, and are supported by staff in managing risk and delivering services aligned with each clients support plan. Family practitioners provide evidencebased interventions and intensive support for the family of the youth client referred to the RR program. Regional Reset Program Bundaberg and Fraser Coast, QLD The Regional Reset Program is a traumainformed, culturally grounded initiative funded by the Qld Department of Youth Justice and Victim Support under the Making Qld Safer Plan. The program stabilises young people, reconnects them with family and culture, and supports positive pathways back into the community. It is a placebased, holistic model offering flexible, culturally safe reset pathways: shortterm intensive livein support and creative and structured group programmes meeting diverse needs and readiness levels of young people aged 817. Regional Reset will divert young people from highrisk environments and provide an opportunity for respite and reset of relationships, behaviours and attitudes. It is intended to help young people demonstrating highrisk behaviour (e.g., substance abuse, aggression or truancy) and their families by providing a short stay of one to three weeks, as well as mentoring and therapeutic supports before and after the stay. The programme focuses on education, selfcontrol, counselling, training and employment skills that include community volunteering, cultural activities, skills, adventure and sport. Services provide culturally responsive engagement with young people and their family or guardian/carer where appropriate, and shortterm aftercare to embed learnings for both the young person and family, as well as support warm referrals to communitybased supports to address ongoing needs. Key Requirements of the Program Targeting young people who require a higher level of care than communitybased programmes can provide, who are moving quickly toward a life of crime. Providing young people with life skills, structure, and selfcontrol they need to get back on track before they become repeat offenders.
