Job Summary: The Open Futures Disability Support Worker provides direct care and assistance to participants with disabilities to help them achieve their personal goals, maintain independence, and participate in the community. The role involves supporting daily living activities, promoting wellbeing, and following participant care plans and NDIS standards. Key Responsibilities: Support participants with personal care, mobility, and daily living activities (e.g., hygiene, meal preparation, dressing). Assist participants in community access activities such as shopping, appointments, recreation, or social events. Follow each participants Individual Support Plan and Positive Behaviour Support Plan (PBSP). Promote choice, dignity, and independence in all interactions. Accurately record progress notes and report any changes in the participants behaviour, health, or routine. Complete incident reports promptly in accordance with NDIS and organisational policies. Maintain professional boundaries and confidentiality at all times. Communicate effectively with participants, families, team members, and supervisors. Ensure a safe and supportive environment by following workplace health and safety procedures. Qualifications & Requirements: Certificate III or IV in Individual Support, Disability, Aged Care, or a related field (preferred). Previous experience in disability, aged care, or community support work Current First Aid and CPR Certificate. Valid NDIS Worker Screening Check and Working With Children Check (if applicable). Valid Drivers License and reliable transport (if community access support is required). Understanding of NDIS Code of Conduct and Restrictive Practice guidelines. Good written and verbal communication skills. Compassionate, patient, and respectful attitude towards people with disabilities. Desirable Skills: Experience working with participants with complex needs or behaviours of concern. Knowledge of manual handling and infection control procedures. Ability to work independently and as part of a multidisciplinary team.
