What you will be doing (Key accountabilities) Engagement and Intake Manage enquiries from prospective new Persons Supported via phone, email, webform and face-to-face for the purpose of acquisition, growth, and retention. Discuss the person''s interest in Melba''s services; collate, receive, and review the individuals needs, ensuring clarity through open dialogue and discussions. Ensure informed consent and understanding of rights and responsibilities. Ensure that in delivering services, Melba and its staff respect and safeguard individuals'' human rights at all times. Planning and Service Coordination Develop and deliver support plans; coordinate services and make referrals to relevant Melba supports and external providers as needed. Communicate with staff, agencies, and external providers on behalf of the person supported. Review, coordinate and update changes to 12monthly service agreements. Provide necessary information, support, and resources for active participation, skill development, and progress tracking, while managing service costs. Relationship Management and Monitoring Conduct structured checkins and reviews with people supported as per guidelines, reviewing progress, support effectiveness, and evolving needs. Coach the person supported working with them to develop capacity and resilience within their networks. Assist the person supported in building and maintaining a resilient network of formal and informal supports. Develop the capacity for the person supported to implement and manage their supports and network independently over time. Support the person supported in enacting their choice and control. Support the coordination of any complaints, incidents, quality of care issues, and feedback to relevant stakeholders. Collaborate with the person and Melba supports to identify and overcome service barriers, ensuring quality service delivery and ongoing monitoring through feedback and service checks. Partner with the Practice Lead to document and maintain relevant information regarding supports and outcomes, including Personal Outcome Measures (POMs). Compliance and Continuous Improvement Ensure a coordinated response to learnings from incident reports, complaints, and quality of care reports. Adhere to legislation, program guidelines, and accreditation standards, ensuring accurate data management and compliance with policies and procedures. Contribute to continuous improvement initiatives by meeting targets and key performance indicators, working within quality frameworks, and attending relevant meetings. Uphold human rights and ensure a safe working environment for all stakeholders, sharing effective practices to build a strong health and wellbeing culture. Stakeholder and Community Engagement Build relationships and partnerships with organisations that assist in creating opportunities for participants who purchase support coordination services from Melba. General Duties Undertake any other additional responsibilities as directed by your manager. Your Background (Skills and attributes) Understanding of services and systems, coupled with effective planning and organisational skills. A good understanding of Positive Behaviour Support (PBS) and personcentred practice. A strong understanding of when and how to engage allied health or other professional expertise to ensure the health and wellbeing of the person supported. The ability to adopt communication techniques that respond to the unique needs of each person. High attention to detail, ability to prioritise tasks, meet deadlines, and work effectively both independently and as part of a team. Quick to adapt to modern technologies, including proficiency in Microsoft Office suite and capacity to learn inhouse database systems. Experience in supporting people with disabilities and their families. Strong interpersonal skills and ability to form effective working relationships with colleagues, management, families, and external bodies. Effective communication skills (language and literacy) with a demonstrated ability to maintain accurate and legible records. Ability to take on responsibility and accountability to drive positive change. Willingness to collaborate and build positive relationships, demonstrating a resilient and adaptable mindset. Can bounce back from setbacks to navigate challenges. Bring a flexible, positive, and selfmotivated mindset to your interactions every day. Certificate IV in Disability is desirable but not mandatory. Requirements A current NDIS Worker Screening and Working with Children Check (we can support you through the process of obtaining one or both checks if you dont already have them). The ability and relevant driver''s license to perform travel and work, as needed. A current First Aid and CPR Certificate. Compliance with OHS&W regulations, legislation, and Policies and Procedures is required. Successful completion of preemployment safety screening checks, mandatory training, and their subsequent renewals is necessary. Occupation Health Safety and Wellbeing (OHS&W) Take reasonable care for their health, safety, and wellbeing and that of others. Observe safe systems of work. Follow OHS&W policies and procedures. Report hazards, near misses and incidents in accordance with the agreed incident reporting system. Contribute to a positive culture in relation to OHS&W and participate in consultative structures.
