Working closely with clinical colleagues, families and community providers, you''ll play a key role in delivering a seamless, empowering care experience.ResponsibilitiesLeading the development and ongoing review of person-centred care plans that reflect each participant''s goals, preferences and wellbeing needsCoordinating care and services across Support at Home pathways, ensuring timely, goal-aligned and proactive supportBuilding strong relationships with participants and their support networks to enable shared decision-making and dignity of riskSupporting participants to understand and manage their Support at Home budgetsWorking collaboratively with clinical Care Partners, multidisciplinary teams and external providers to ensure integrated, high-quality careMaintaining clear, timely communication with families, carers, GPs and allied health partnersEnsuring risk identification, incident response and documentation meet organisational, compliance and aged care quality standardsAbout YouYou''re someone who genuinely cares about supporting older people to live well at home.
