Position overview We are seeking an experienced Community Worker to deliver structured community and welfare support services to clients in home and community settings. This role focuses on assessing needs, coordinating supports, delivering participation programs, and connecting clients with appropriate services and resources to achieve their individual goals. Key responsibilities Assess individual client needs, circumstances, strengths, and support requirements through direct engagement and ongoing review Plan, develop, and implement individual support strategies that promote independence, inclusion, and wellbeing Support clients to access health, housing, education, training, employment, and community based services Assist clients to participate in community, social, recreational, and daily living activities aligned with their goals Monitor client progress and wellbeing, identifying changes in needs and responding appropriately Liaise with families, carers, community organisations, and service providers to coordinate and integrate supports Provide information, guidance, advocacy, and referrals to relevant welfare and community services Maintain accurate case notes, progress records, and service reports in line with organisational and funding requirements Contribute to service planning, outcome reporting, and continuous improvement activities Deliver services in a professional, respectful, and culturally sensitive manner at all times Skills and experience Minimum two years of relevant experience in community services, disability support, aged care, or a related welfare role Demonstrated experience assessing needs, coordinating supports, and delivering community based programs Strong communication and interpersonal skills, with the ability to build effective relationships with clients and stakeholders Ability to work independently in community settings and collaboratively within a team Organised, adaptable, and reliable approach to work Qualifications Certificate III or IV in Community Services, Individual Support, Disability, Aged Care, or a related field Diploma in Community Services or equivalent is highly regarded Requirements Valid Queensland driver licence and access to a reliable vehicle NDIS Worker Screening Clearance Current First Aid and CPR certificate Manual Handling Certificate Working with Children Check Evidence of Flu Vaccination Right to work in Australia Whats in it for you Flexible shifts to support work life balance Diverse community based work settings and experiences Supportive and inclusive team culture Ongoing training and professional development opportunities Opportunity to make a genuine and lasting impact in your community If you have the experience, qualifications, and commitment to supporting people to live active and connected lives, we welcome your application to join Soulmate Care as a Community Worker.
