This pivotal leadership role combines clinical nursing, care coordination, and relationship management to support elderly, frail, and disabled clients with person-centred, high-quality care.About the RoleAs a Clinical Care Coordinator, youll be instrumental in delivering consumer-directed services that promote independence, dignity, and wellbeing.
