Aboriginal Child & Youth- Health Pathway NavigatorAboriginal Child & Youth - Health PathwaysNavigatorDo you wantto make a real difference in the lives of Aboriginal children and young people?We areseeking a committed and culturally responsive person to take on the role of AboriginalChild & Youth - Health Pathways Navigator. You will support Aboriginalchildren and young people in out of home care to access timely, culturally safe,health care and wellbeing supportThis roleis about more than coordination its about wrap around care, advocacy,cultural connection and walking alongside children, families and carersIn thisrole, you will:Identify and respond tounmet health and wellbeing needs of Aboriginal children in careCoordinate culturallysafe health assessments, including Aboriginal health checks and screeningprogramsDevelop and reviewculturally appropriate care plans alongside children, families and carersSupport families to accessservices and navigate complex care systemsLiaise with ChildProtection, carers and service partnersMaintain accurateclient and program documentationWe arelooking for someone who brings both professional capability and heart to theirwork.
