Job Description

As a Care Partner, you will play a pivotal role in delivering high-quality, person-centred care to both potential and existing clients. You will lead and coordinate all aspects of client care, managing the overall client experience while working closely with our dedicated office team to ensure seamless service delivery. Based in our Scoresby office, you will work with a local client base and regularly visit clients in person, allowing you to build meaningful relationships and deliver care that is genuinely tailored to each individual. With a reduced caseload, youll have the time and capacity to focus on quality outcomes, advocacy, and the best interests of your clients while supporting the achievement of our business goals. Key Responsibilities Build trusting relationships with prospective and existing clients, their families, and representatives. Conduct regular quality assurance visits (in person or via phone) to monitor care, address changing needs, and recommend additional services as required. Collaborate with clients, families, Caregivers, and other stakeholders to ensure care plans remain aligned with evolving needs and preferences. Maintain strong local knowledge and networks by staying informed about community services and engaging with health providers and referral sources to promote available support options. About You Tertiary qualification in a health-related field is desirable (e.g. social work, allied health). 2+ years of experience working in a similar role within aged care. Strong organizational skills with the ability to manage time effectively, multitask, and adapt to shifting priorities under pressure. Excellent written and verbal communication skills, with accurate data entry and proficiency in Microsoft Office.
