Aboriginal and Torres Strait Islander Senior Health Worker Sunshine Coast Hospital and Health Service Nambour QLD An exciting opportunity exists for a Senior Health Worker to join the Aboriginal and Torres Strait Islander Health Directorate. The Aboriginal and Torres Strait Islander Senior Health Worker role functions within the Preventative Health Pathways program, which is a multidisciplinary team that promotes integrated service provision across the continuum of primary health care, consistent with principles of client self-management and Hospital Avoidance. Sunshine Coast Health is in southeast Queensland and extends through the coastal and hinterland areas from Caloundra in the south and Gympie in the north. Key Responsibilities include: Contribute to the development, implementation, and evaluation of relevant and appropriate education resources for Aboriginal and Torres Strait Islander people to improve health literacy, access, and equity to available health care services. Exercise accountability, responsibility for quality and quality systems of Actively participate in the Health Service professional development/performance process relevant to your Contribute to Closing the Gap and Health Equity Key Performance Indicators as underpinned by the Queensland Health Making Tracks toward closing the gap in health outcomes for Aboriginal and Torres Strait Islander Queenslanders by 2033. Remuneration value up to $128,706 p.a., comprising salary between $98,641 $112,805 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HWF4).
