Central Gippsland Health Service Sale VIC Central Gippsland Health (CGH) is a sub-regional health service and the major provider of health and aged care services in the Wellington Shire. CGH is located in the City of Sale which is 215 km east of Melbourne. The City of Sale is an ideal access point for the magnificent Gippsland Lakes, 90 Mile Beach and the Victorian Alps. CGH provides acute care as part of an integrated service delivery model across Wellington Shire and beyond. The service provides beds across three campuses (Sale, Maffra and Heyfield). Acute services include a full-time emergency department, critical care unit, neonatal critical care unit, three operating theatres, day procedure unit, oncology and dialysis services in addition to general medical and services and subacute services including rehabilitation. CGH is also a leading teaching hospital, affiliated with Monash University''s School of Rural Health and provides placements and clinical experience for students. About Department Wilson Lodge is a residential aged care facility with 50 beds. Wilson Lodge caters for emergency respite, shortterm planned respite as well as longterm permanent placement to meet the individual needs of the community to which it serves. All residents are welcome regardless of their care needs. Wilson Lodge staff work with individuals to provide high quality care in line with aged care quality and safety commission guidelines and the national aged care standards. About the Role To provide high quality personal care to residents. To work with the Aged Care Service Manager, Assistant Nurse Unit Managers, Allied health, Medical staff, families and residents to tailor personentered care that is highly individualistic. To maintain standards in line with the Aged Care Quality and Safety Commission and Aged Care Standards. Duties Establish a relationship between the resident and yourself in order to achieve the best possible outcomes for your resident. The RN may be required to work with residents who are at risk of COVID19 or have tested positive for COVID19. Support resident care in accordance with the best interests of the resident. Apply knowledge and skills to undertake systematic assessments of residents, develop and document the plan of care. Implement the plan by providing direct care to residents. Through the use of critical and reflective thinking skills evaluate the effectiveness of care towards meeting expected resident outcomes. Coordinate and support effective discharge planning processes which commence on admission for residents staying in the facility in shortterm respite arrangements. Recognise a change in the residents condition, in particular deterioration, take necessary action and document variation in the care plan. Balance competing priorities and work demands and identify and respond appropriately to critical and urgent clinical needs. Work in a highly organised manner. Support the coordination of admissions, transfers and discharges communicating with the incharge nurse proactively. Participate in interdisciplinary assessment and service delivery planning processes consistent with and in support of care coordination across the care settings. Escalate concerns regarding safety, quality of care and risk issues to the incharge nurse. Support patient flow requirements as requested to meet organisational and patient care priorities. Ensure timely clinical documentation is at the highest standard. Lead a culture of excellence, ethical conduct and interdisciplinary collaboration. Support the development of, complies with and ensures organisational policies and procedures are followed and that clinical standards are constantly monitored and met. Ensure that the incharge nurse is informed of any practices that may inhibit the delivery of a high standard of resident/client care. Support the ongoing service improvement and strategic positioning of the facility by creating an environment that supports creativity, innovation and collaborative decision making within the service and through collaborative projects or endeavours across services and/or departments; that encourages people to identify inefficient or ineffective structures, systems and processes and supports them to find and implement solutions; that values excellence in customer service and continuous quality improvement. Develop a culture within the ward/unit that enables staff to understand the need to comply with quality and safety guidelines and meet quality accreditation standards. Support a culture that values and supports continuous improvement. Participate in complaint management processes. Ensure all reporting requirements as required by the incharge nurse and the organisation are completed in a timely manner, inclusive of the use of information technology with regard to the application of health care technology. Utilise effective communication skills with all stakeholders. Participate in CGH and team meetings, working groups and committees as appropriate. Provide support and facilitation of learning opportunities to undergraduate students from all disciplines.
