About the role Be responsible for coordinating tailored home care services that support older clients to live well at home, while ensuring care is delivered safely, respectfully and in line with Carinitys values. You will work collaboratively with families, clinicians and care staff, uphold compliance requirements, and play an active role in fostering client satisfaction and positive care outcomes. Your day-to-day will involve Responsible to implement, review and maintain individual support plans, budgets based on client goals, needs and My Aged Care assessments Coordinating and monitoring the delivery of high-quality, client-focused care services within allocated budgets Conducting assessments, recording referrals and updating client information in a timely and accurate manner Liaising with clients, families, healthcare providers, ACAT teams and external stakeholders to ensure seamless high quality service delivery is provided Supporting, mentoring and coordinating Lifestyle Carers, including assisting with training, onboarding and guidance Contributing to recruitment and ongoing development of home care staff Actively participating in continue improvement, quality and compliance. About you You bring warmth, patience and genuine empathy to your work, with a commitment to delivering exceptional care to older people. Confident in coordinating services and collaborating with others, you build trusting relationships with clients, families and colleagues alike. Your strong communication skills and client-first mindset ensure you consistently support wellbeing and independence.
