Job Description

The Federal Government appointed Country SA PHN to enhance the delivery of focused and appropriate health services to the rural communities of South Australia. The goal of the organisation is to improve the efficiency and effectiveness of health services for people particularly those at risk of poor health outcomes, improve the coordination of health services, and increase access and quality support for people. Job Description The Role The HealthPathways Clinical Coordinator role supports the development, implementation, and ongoing improvement of HealthPathways across South Australia, an online clinical information and referral portal used at the point of care to support assessment, management, and referral of patients. Working in partnership with Country SA PHN, Adelaide PHN, Wellbeing SA, and Streamliners NZ, the role coordinates the localisation of clinical pathways, supports stakeholder engagement, and contributes to consistent care delivery, improved referral clarity, and a better patient journey across the South Australian health system. The position requires collaboration with internal teams, GP Clinical Editors, health service providers, and external partners to ensure HealthPathways SA remains a trusted, highquality clinical resource. It is expected that this position will remain flexible to meet the changing needs of the organisation and the community. Desired Skills and Experience Essential Criteria Qualifications: A tertiary qualification in a healthrelated field and/or extensive experience in healthrelated project management or program delivery. Experience/Knowledge/Skills/Abilities: Experience in health program management, coordination, or service redesign. Highlevel oral and written communication, including report writing and stakeholder engagement. Ability to develop relationships and work collaboratively across sectors. Experience liaising with general practice, senior clinical leaders, and service providers. Strong organisational skills with ability to meet competing deadlines and manage multiple tasks. Ability to identify outcomes, set priorities, and solve problems effectively. Experience working autonomously and within multidisciplinary teams. Understanding of the primary health care sector in South Australia. Understanding of the need for integrated primary and secondary care pathways. Proficiency in MS Office software and confidence using digital communication tools. Capacity to adapt to changing priorities and program needs. #J-18808-Ljbffr
