About the Role What you will be doing - Supporting Care Partners is the provision of consistently highquality services that are customer focused, fulfil requirements and exceed expectations to ensure that NovaCare participants are satisfied and retain NovaCare as their service provider of choice. Developing and maintaining good relationships with relevant staff to ensure optimum outcomes for participants. Maintain accurate, timely care notes and ensure documentation supports outcomes under the Strengthened Aged Care Quality standards. Ensure that service requests are submitted accurately and in a timely manner, work collaboratively with the scheduling team to resolve any scheduling issues or concerns. Work collaboratively with Care Partners and Clinically Qualified Care Partners to follow through on care instructions and service coordination. In line with escalation protocols promptly elevate clinical concerns or changes in participant condition to the Care Partner or Clinical Care Partner. Identify opportunities for innovation and take personal responsibility for continuous improvement and quality of own work. Maintain own professional development in line with the requirements of the role in accordance with current standards. About You What you need to succeed - A passion for exceptional customer service, and a motivated to achieve exceptional customer satisfaction scores through consistently meeting and exceeding expectations. Ability to build rapport, establish relationships and adapt communication style to suit different audiences. Demonstrated ability to balance conflicting priorities in order to manage workflow, ensure the completion of essential projects, and meet critical deadlines. Sound digital skills, including data entry and CRM/care system use. Willingness to be responsive and flexible, demonstrated recognition of the demands of a customer driven service delivery model that empowers choice. Resilience, adaptability and initiative in a fastpaced environment. Minimum Certificate III or above Aged Care, Community Services, or relevant fieldor equivalent experience. Experience in administrative support, care services, or clientfacing roles in aged care, disability or community settings. Demonstrated administration experience and attention to detail.
