ABOUT THE ROLE Your responsibilities may include but are not limited to: Providing care management support in accordance with the new Aged Care Act and CSAH Policy & procedures, within set time frames, according to set targets and individual participants budgets. Liaising with internal and/or external services/associated providers to improve the implementation of care plans for participants and to provide services where required. Developing and reviewing comprehensive person-centered care plans in consultation with participants, their supporters and relevant stakeholders within individual participants budget parameters. Maintaining effective links with relevant professional networks (e.g ACAS, GP and community services) to improve the participants experience when needed. Regularly provide support and advice to participants and their supporters to enable them to make informed decisions, contribute to their care plan and evaluate the effectiveness of current care plans. Advocating on behalf of participants where required to ensure required services are delivered Investigating and responding in a timely manner to participants inquiries, concerns or complaints and ensuring that participants are advised of the agreed level of services and any changes to plans. Promote CSAH services and provide information to prospective participants. Willing to work in an office based environment Monday to Friday, on a full time basis, with visits to clients in their homes; part-time will be considered for the right applicant. Demonstrated experience in developing and implementing strategic operational plans and record of driving change and compliance. A clinical background as a Social Worker/EN/RN/Allied Health Practitioner/Care Manager is highly desirable Candidates with non clinical care management experience will be considered for this position.
