Role Summary As a Support Coordinator, you will work with NDIS participants and their networks to identify goals, access services, and foster independence. Youll oversee service delivery, engage with providers and participants, and explore new support options. Key Responsibilities Implement supports from NDIS plans including formal, mainstream and funded services. Assist participants to understand their plan funding, negotiate services and monitor delivery. Manage timelimited specialist coordination for highrisk situations, and provide crisis support when needed. Use a strengthbased, personcentred approach to develop support plans. Build and maintain professional relationships with participants, families/carers and advocates. Resolve client issues and complaints fairly in accordance with policy. Maintain contact with participants, manage your caseload, and record interactions accurately. Stay updated with community resources and coordinate referrals. Foster effective partnerships with service providers, the disability & mental health sectors, and the NDIA. Comply with relevant legislation, codes of conduct, NDIS policies, billing requirements and documentation standards. About You Strong organisational, administrative, timemanagement and communication skills. High attention to detail, excellent problemsolving and decisionmaking ability. Cultural sensitivity, inclusivity and ability to build trust and rapport. Comfortable managing change, identifying opportunities for innovation, and handling financial aspects such as billable hours and budgeting. Ability to work independently with your own caseload while contributing to a culturally diverse team. Qualifications & Experience Relevant tertiary qualification in; Disability, Individual Support, Community Services, or substantial demonstrated experience. Minimum one years experience in a similar role, with experience in complex support needs. Knowledge of the NDIS and disability sector, including how to optimise supports for participants. Familiarity with human rightsbased, personcentred, positive behaviour support and active support approaches. Ability to coach, mentor and develop a team. Proficiency in Microsoft Office. Current NT unrestricted drivers licence. National Police Check. First Aid & CPR certificate. Valid NDIS Worker Card. OCHRE Card. Hands on People is an Equal Opportunity Employer.
