Position Overview Care Partner plays a pivotal role in supporting older people, their carers, and service providers to work collaboratively to achieve positive health and wellbeing outcomes. This position ensures: High-quality, culturally appropriate service delivery Strong coordination between care teams, Registered Nurses, and management Compliance with legislative, regulatory, and organisational requirements The role is central to maintaining Sri Om Cares commitment to culturally specific, safe, and person-centred care. Key Responsibilities Work collaboratively with management and Registered Nurses (RNs) to ensure care plans are clearly documented, clinically appropriate, aligned with client goals, and formally approved. Maintain and promote Sri Om Cares culture-specific service delivery model in line with current aged care standards and legislation. Addressing feedback, concerns, and complaints Managing staff issues in a culturally sensitive manner Coordinate the delivery of quality, person-centred care that meets both clinical and cultural needs of clients. Conduct client and family meetings and explain Sri Om Cares services and processes in clients native languages where required. Allocate and deploy support workers based on: Skills and qualifications Cultural and language compatibility Personality and client preferences Ensure compliance with Work Health and Safety (WHS) requirements, including incident reporting, hazard identification, and risk management. Escalate client complaints, incidents, and non-compliance issues to management promptly and ensure accurate system documentation. Proactively identify and manage potential staffing shortages, ensuring continuity of culturally appropriate care. Perform administrative duties as required, including: Answering phones and emails Supporting recruitment and interviews General coordination and office tasks Build strong relationships with clients, families, workers, and stakeholders to support diverse clients in achieving their care goals. Manage and maintain compliance documentation for both clients and care workers in line with Government Standards and organisational policies. Ensure all documentation is accurate, complete, and maintained within Sri Om Cares databases and software systems, in accordance with legislative and organisational requirements. Eligibility & Essential Requirements Nursing Student with minimum 2 years experience in Aged Care, or Certificate IV in Aged Care / Diploma of Nursing Strong understanding of person-centred and culturally responsive care Proficient computer and documentation skills Fluency in Hindi and/or Punjabi (essential) Knowledge of the Social, Community, Home Care and Disability Services Award (highly desirable; training can be provided) Valid drivers licence and own vehicle (Essential) Why Join Sri Om Care?
