St Louis Care is made up of four specialist service areas: St Louis Homes NDIS-approved provider for younger onset dementia St Louis Home Care Government-approved and private support at home care provider St Louis Nursing Home Boutique residential aged care St Louis Allied Health Covering both Metro and Fly in Fly Out (FIFO) allied health services supporting South Australian communities Role: Care Partner (Victor Harbor | Full-Time) As a Care Partner, You Will Provide personalised, highquality support to clients in their homes. Develop, implement, and regularly review personcentred care plans and budgets based on client goals, assessments, risks, and support needs. Codesign care plans with clients, families, carers, and internal teams to ensure consistent, quality care. Conduct comprehensive risk assessments and support safe care transitions. Promote wellness, reablement, health education, and informed decisionmaking. Support clients to understand and manage budgets, cocontributions, and approved services. Monitor and approve expenditures, process claims/invoices, and manage consumables in line with financial policies. Ensure all support remains within funding guidelines. Work collaboratively within a closeknit team in a fastpaced environment. Communicate effectively with clients, families, and colleagues. Maintain accurate client notes and documentation using Microsoft systems and internal software. About You You are someone who thrives in a team setting but can also work independently with minimal supervision. Youll Bring Strong communication and interpersonal skills, including experience with CALD communities. An outgoing, approachable, and supportive working style. Proficiency in Microsoft Office (Word, Outlook, Excel) and the ability to use internal software systems. Ability to problemsolve, plan, and manage competing priorities. Report writing and basic financial management skills. Knowledge of aged care services, advocacy, and casework procedures. A genuine passion for supporting older people to live well at home. Essential Requirements Diploma of Nursing or higher; OR relevant diplomalevel qualification. Certificate III in Individual Support or equivalent (if applicable for direct care duties). Minimum 1 year of experience in aged care, home care, or community services. Current National Police Clearance. Valid and unrestricted drivers licence (minimum P2).
