With the rollout of reforms under theSupport at Home Program, this role offers the opportunity to step beyond traditional case management and into leadership in action.A company vehicle is provided for business travel between Bundall, Tweed Heads, and surrounding suburbs.About the RoleAs a Care Partner you will be responsible for coordinating care services under the Support at Home framework, ensuring clients are empowered to make informed choices and maintain independence.
