You will be working collaboratively with participants and key stakeholders to provide comprehensive assessments and individualized case management. You will play a key role in promoting the participants goals and wellness outcomes.ResponsibilitiesConducting HCP assessments and reviews with participants and their families / registered supporters in their homes, in accordance with the HCP guidelinesNetworking, attending industry events and generating leads as well as collaborating with Comlinks marketing and customer engagement teams;Maintaining high quality assessment and care planning documentation;Fostering respectful relationships with participants to fully understand their needs and ensure our services are reflective of person-centred practice and the Strengthened Aged Care Quality Standards;Providing comprehensive case management, ensuring the day-to-day wellbeing of our clients through case work, advocacy and planning;Coordinating any necessary allied health and nursing partners to reassess participants and liaise with clinicians and multidisciplinary teams regarding appropriate provision of supports;Managing participant quarterly budgets to ensure no overspends and budget is achieved;Meeting regularly with participants and their decision makers to continuously monitor assess and respond to changing needs;Working collaboratively with other team members, community organisations, and service agencies to ensure the best outcome for the participant and their care; andManaging participant feedback and complaints regarding service delivery in line with internal policies and ACQS requirements, undertaking remedy actions to address issues.Other things you might like to know:Travel is required, including regular client visits in Hervey Bay (kilometre reimbursement offered);Fully paid compliance checks offered, including NDIS Worker Screening.QualificationsHas a (Minimum Diploma) qualification in Nursing, Community Services (Case Management), Human Services, Health Sector Management, Ageing Studies or Disability / Aged Care, and ideally a minimum of 3 years experience in the industry.Is relatable and engaging with highly developed interpersonal skills, communication skills and emotional intelligence with the proven ability to foster relationships and connect with peopleIs highly organised with exceptional time management skills and can work autonomously while managing competing demandsHas a demonstrated understanding of the Aged Care reform, Strengthened Aged Care Quality Standards, the sector and people who are aging in the communityIs experienced in case management preferably within the home and community care sector with advanced knowledge of person-centred careHas experience in service development, and driving continuous quality improvement and practice and service excellence aligning to the Strengthened Aged Care Quality standardsHow to applyBe creative!
