About the Position: We are currently hiring Personal Care Assistant, Disability Carer and Disability Services Officer. You will be required to provide high-quality services to the people we support in their own home, disability accommodation (SIL) and in the community. 1. Personal Care Assistant Position Summary: The Personal Care Assistant provides essential support to individuals who require assistance with daily living activities due to age, disability, or illness, helping them maintain independence and quality of life in a safe and supportive environment. Duties and Responsibilities: Assisting clients with personal care activities such as bathing, grooming, dressing, toileting, and mobility. Preparing and serving meals according to dietary requirements and assisting clients with feeding when necessary. Supporting clients in transferring and ambulation using appropriate mobility aids and safe handling techniques. Providing emotional support and companionship to enhance clients mental and emotional well-being. Observing, recording, and reporting changes in clients health, behaviour, or physical condition to supervisors or healthcare professionals. Administering or assisting with medication under supervision and in accordance with care plans. Performing domestic duties such as cleaning, laundry, and maintaining a hygienic living environment. Encouraging and supporting clients to participate in social, recreational, and community activities. Maintaining accurate care documentation and progress notes in line with organisational and regulatory requirements. 2. Disability Carer Position Summary: The Disability Carer provides day-to-day support and personal care to individuals living with physical, intellectual, or developmental disabilities to enhance their independence, dignity, and participation in the community. Duties and Responsibilities: Assisting clients with daily personal care tasks such as bathing, dressing, toileting, and grooming. Supporting clients in mobility and physical activities using safe manual handling practices. Implementing individual support plans and behaviour management strategies to meet clients goals and care needs. Encouraging and facilitating participation in educational, social, and recreational activities. Providing emotional and social support to promote clients wellbeing and social inclusion. Monitoring and documenting clients progress and reporting any changes in condition to supervisors or case managers. Assisting clients to develop independent living skills, including meal preparation, cleaning, budgeting, and personal organization. Administering medication and first aid in accordance with established procedures and guidelines. Liaising with healthcare professionals, families, and other service providers to ensure coordinated and consistent support delivery. 3. Disability Services Officer Position Summary: The Disability Services Officer is responsible for planning, coordinating, and delivering programs and services that support people with disabilities to achieve greater independence, inclusion, and participation in community life. Duties and Responsibilities: Assessing clients needs and developing individualised care or support plans in consultation with families and other professionals. Coordinating and implementing programs to develop clients vocational, social, and life skills. Providing direct support, counselling, and advocacy to individuals and families to access community and government resources. Monitoring and evaluating clients progress and adjusting care or program plans accordingly. Supervising and mentoring support staff to ensure the consistent and high-quality delivery of care and services. Coordinating with allied health professionals, government agencies, and community organisations to ensure holistic client support. Promoting community awareness and inclusion of people with disabilities through outreach and education. Preparing detailed case notes, progress reports, and compliance documentation in accordance with policy requirements. Ensuring all services are delivered in line with relevant legislation, NDIS standards, and organisational guidelines.
