Physiotherapist - Home and Community Care Discover a career where care goes both ways! This role involves assessing, planning and delivering individualised/group physiotherapy services and activities to improve the wellbeing of clients in their own homes as well as running group sessions across our Wellness Hubs. Location and Employment Location Southern Central Coast (Gosford, Kincumber and surrounds) Employment type Permanent Full Time or Part Time Pay up to $54.50 per hour (based on experience) + up to $18,550 tax free + superannuation Sponsorship available for candidates with AHPRA seeking PR Day to Day Responsibilities Conduct comprehensive assessments of clients'' physical and functional abilities Develop and deliver individualised or group treatment plans based on clients needs, goals, and preferences Implement evidencebased physiotherapy interventions to address mobility issues, pain management, and other agerelated conditions Regularly assess and report on the physical and psychosocial capabilities of clients to the care management team Maintain accurate records, exercise sheets, assessment reports and progress notes Make recommendations for exercise programs, mobility equipment and safety at home and refer to other disciplines as required Uphold ethical and professional standards in accordance with regulatory guidelines Collaborate with interdisciplinary teams and provide holistic care Educate clients and families on preventive measures and exercises Participate in team meetings, case conferences, training sessions and continuous improvement activities Engage respectfully with clients, families and culturally diverse team members Qualifications and Experience Bachelor or Post Graduate Degree in Physiotherapy and current AHPRA registration Excellent interpersonal, verbal, and written communication skills Ability to manage compliance and achieve timely outcomes Experience working within a multidisciplinary team Effective time management skills to prioritise caseload Computer literacy Knowledge and experience in health, aged and/or disability services Current Drivers licence and registered insured motor vehicle What We Can Offer You Paid travel time & KM reimbursement Notforprofit tax benefits so you pay less tax Access to professional development and clinical support (up to $2,000 CPD allowance per year) Discounted gym benefits (Fitness Passport) Paid parental leave Long Service leave after 5 years 40+ locations across NSW and QLD for relocation A valuesbased organisation welcoming applicants from all faiths and backgrounds Apply Online We know you have many choices of employer choose Catholic Healthcare where you can make a positive impact and be well rewarded. #J-18808-Ljbffr
