About the role This exciting role offers the chance to connect with urban, rural, and remote communities through our innovative Diabetes Alliance Program Plus (DAP+) Medibus and Integrated care model between diabetes specialists and the primary care sector. Responsibilities Provide expert podiatry care and advise for people with diabetes. Develop and facilitate education to primary care and allied health clinicians at our DAP+ led education events and Lower Limb Community of Practice (CoP). Travel across the region with the DAP+ Medibus team when needed to support patients, General Practice and Allied Health staff. Contribute to service planning, evaluation and quality improvement initiatives. A typical week may include supporting clinic-based care, outreach visits on the DAP+ Medibus, and collaborating with HNE Health and primary care stakeholders to enhance patient outcomes, service mapping and facilitating education to meet the needs of local communities and healthcare providers. Where youll be working The DAP+ is a collaborative program designed to improve diabetes care through specialist outreach and integrated models of care within the primary care sector. Youll be based at Wisteria House, James Fletcher Campus, and work across urban, rural, and remote locations via our DAP+ Medibus. Our skilled multidisciplinary team includes Endocrinologists, Diabetes Educators, Dietitians, Managers and Administration Officers, all committed to excellence and cultural respect. About you We are looking for someone with experience in diabetes-related podiatry care someone who is able to work independently, as well as a part of a multidisciplinary team, and has the capacity, knowledge and experience to supervise fellow students and junior medical staff.
