Youll play a vital role in improving quality of life, advocating for autonomy, dignity, and respect, and fostering a nurturing environment. Key Responsibilities - Client Assessment & Monitoring: Conduct assessments and monitor residents health, responding promptly to changes. - Care Coordination: Collaborate with healthcare professionals to create and implement individualised care plans. - Clinical Care Delivery: Manage medications, wound care, and continence care. - Documentation: Maintain accurate medical records and reports.kli> - Team Support: Provide guidance and support to Care Support Workers and deliver training to others.
