You will collaborate within a multidisciplinary team that includes Palliative Care, NurseLed Clinic, Wound Management, Child and Family Health Nursing, General Practitioners and a wide range of outreach services such as Diabetes Education, Dietetics, Aged Care, Paediatric Therapy and Womens Health. This integrated approach ensures holistic, patientcentred care that meets the diverse needs of the community. What youll be doing As a Community Registered Nurse you will provide a high standard of personcentred care to clients requiring wound management and palliative care support. Care will be delivered across both clinicbased settings and in patients homes, ensuring flexible and responsive service delivery. You will apply evidencebased practice and critical thinking to assess, plan, implement and evaluate individualized care plans. Working closely with the multidisciplinary team, you will contribute to improving patient outcomes while supporting patients and families through complex and often sensitive care needs. This position is supported by the Nurse Unit Manager, with additional specialty support provided by the Wound Management Specialist and Clinical Nurse Educator. Education and training opportunities will be encouraged and supported where they align with the core business of the Community Nursing Team.
